Does anybody know of a good website where I can find rates for incidence/prevelance of certain diseases?

It's for a school paper.

NYS Department of health website (google it) There is a statistics page by county.

MMWR (morbidity and mortality weekly reports) regularly publishes data on reportable diseases. It's put out by the CDC.
