UTIs and elevated kidney levels in cats
So, for the 2nd time in my 8 year old cat's life, he had a severe UTI and was blocked. I took him to the vet yesterday and the urine that was coming out of him was almost all blood, it was horrible! It wasn't like that last time which was 4 years ago. Anyway, I thought all was going to be ok.
Now, the vet is telling me that his kidney levels are extremely high and until he sees them come significantly down, he can't really say much to me about his prognosis. I am freaking out here, i really am. I don't know what to think, I thought he would be in the hospital for a few days, get the catheter, get the IV and all would be well. Now I am scared to death his kidney levels won't come down, then i don't know what will happen.
I am also racked with guilt because I am thinking if I had brought him in sooner, it wouldn't be this bad (he started exhibiting weird signs on Friday and I called the vet on Saturday to bring him in and was told i couldn't bring him until Sunday, i should have forced it or just walked in!!). i feel sick to my stomach and can't believe I have to wait until tomorrow to get more news. I am going to the hospital tonight to see him, they did say he is eating which is a good sign because he hadn't been.
Anyone ever been in this situation before and had a happy outcome?
Oh, and please pray for him and us.

I feel so bad for both you and your cat. I can't be of much help except to tell you about the 4 times I have been in this situation (2 different cats.)
Cat #1: 2 yrs old, blocked, bloody urine (more blood than urine) high kidney levels. He was kept at the vet a few days, catheter, antibiotics, the usual. Came home fine...two days later ..blocked ..it happened again. Back in hospital, catheter, antibiotics cleared....NOT. Came home, 4 days later, another episode. Finally the vet offered surgery...basically removing his penis, which is where the crystals were blocking the flow of urine and seemingly turning him into a female. Never a problem again and he lived to 16 when he had multiple medical problems.
Cat #2: 11 yrs old. Never blocked but did have severe kidney infections.
First one at 11 yrs of age. Antibiotics, Sub Q fluids, special diet. three months later it happened again....this time the kidneys shut down completely and it was kinder to let him go.
If I had to do it over again, I think I would have insisted on more frequent urine cultures to catch the infection at the start (for cat #2) rather than waiting as I did for the cat to become symptomatic. Would that have changed the outcome? Probably not, but it might have made ME feel better.

Poor kitty. This happened to my mom's cat once. The vet suggested we discontinue all food with any kind of fish in it as the magnesium seemed to exacerbate (cause?) the blockages. We did that and he never had another episode.

Oh, Sloper I am so sorry to hear that. I have had a little experience with UTIs, but nothing as serious as that. Hopefully your vet will give you some advice on prevention techniques for future outbreaks, but I know that in some cases they just don't know what causes chronic UTI in some cats. Oh, and don't beat yourself up - kitties in particular are absolute pros at hiding any discomfort due to something being wrong until they are absolutely unable to do so anymore. I'm really hoping this has a positive outcome and my thoughts are with you.

Wow, thanks for the info! the info on Cat #1 scares the beejeesus out of me! i read about that surgery, wondering if that is what they will offer me down the road. But, I bet it's expensive! As it is now, the vet estimate for his hospital visit for this is between $1010.00 and $1240.00! I can't imagine what they would want for that surgery you are talking about!

And, to everyone else, thanks for the well wishes. they say it is good that he hasn't been blocked in 4 years and he does eat only prescription food, so, this is odd that it has ahppe3ned again to him! the vet said they might need to change the prescription food to a different type of prescription.

I have no clue what the surgery would cost today. I paid $1200 for it in 1989. That was for the surgery alone, not the prior hospitalizations, which was a lot of money at that time. People thought I was totally nuts. My 11 y/o, whose kidneys shut down this past November, had vet bills amounting to $4000 for his two episodes three months apart.
I hope all turns out well. -

Holy crap! that is what this hospitalization with IVs, catheters, etc. is costing us. if they wanted to do that too, and considering that is what you paid 20 years ago (so, it must be double that now!)-there is no way that would be affordable!

I am sending good thoughts to and for your cat. I'll be watching here tomorrow for an update on the kidney levels. Think positive.
I doubt the perineal nephrostomy will be required since he was last blocked 4 years ago. If the kidney function returns to normal and he does not become blocked over and over in a short period of time there would be no indication for the surgery. Keep in mind that my cat #1 became blocked repeatedly only days after treatment.
Even if the kidney function does not return totally to normal, your cat can live for several years receiving subcutaneous fluids, at home, administered by you. It is a simple procedure. Google Feline Sub Q fluids and look at the procedure. Your vet will teach you how to do it. -
Thanks again Tnyc (and everyone else), for all the info! Here it is almost midnight and I am sitting on the computer trying to read up on elevated kidney levels and the like. I am glad to know though that even if, God forbid, his levels don't come down by tomorrow, that there are options other than surgery and death for him. all I have been worrying about is the idea that, God forbid, the levels don't come down and the only option would be ridiculously expensive surgery or death. Glad to hear that is not the case.
We went to visit him tonight, me and my children. He looked so much better than he did yesterday! And the urine line coming out of him while still tinged with blood was so much better than yesterday where everything coming out of him was pure blood. He even licked and playfully bit my oldest, whom he adores! I hate to get my hopes up too much though, he did look good.

LTS- after all your reading last night, you should be the resident expert on the kidney function of cats! It is a really good sign that the blood in his urine was less, and that he is alert and playful. That is an indication that the antibiotics are working.
When this is over, suggest to your vet that periodic urine cultures be taken (at some steady frequency) for a while. They are taken with a long, fine needle through the lower abdomen and into the bladder. The urine is cultured and if bacteria is present oral antibiotics can be given. This will save your kitty from the trauma of a serious illness and hospitalization and save your pocketbook from trauma also.

OOh! that is a good idea Tnyc! We have done urine cultures on him but only at his once per year check ups. Maybe they do need to be done more often-that is a very good idea.
i won't know anything until this afternoon-I am going to call the vet around 1 pm (if they haven't called me by then) to see if they have results. -
Awww - poor kitty (and poor you). My 17 year old was recently in the hospital for 4 nights (kidney failure) which cost $1200, was released and put on sub fluids, then started getting really down...turned out his potassium was dangerously low...went on a potassium regime...and he's doing amazingly well.
I don't know who your vet is but if this is your regular vet you should have insisted (i'm not judging you...don't want that statement to come out that way)...My baby had an appt for the following night but I called the night before because he was soooo bad...turned out if I hadn't gotten him in, he would have been dead.

Stupid internet been down all evening.
Anyway, great news!! spoke to the vet earlier today (BTW, we go to Park Slope Veterinary on 5th Avenue, have been going to them for 6 years now), and his levels came down a lot! His BUN on Sunday was 218 and now is 118 and his creatinine was 15.8 on Sunday and is now 5.1!
So, he is doing good and while his levels still aren't at normal, they are going in the right direction! We went to see him again tonight and he was so much better even from yesterday, he even tried to roll over on his back for my oldest to rub his belly. But, with the cone collar and tubes, he wasn't able to. But, definitely perkier and more himself today.
They are going to pull the catheter either tonight or tomorrow and then keep him there for 24 hours after the catheter is pulled just to make sure he doesn't re-block. When they send him home, he will have 2 or 3 different meds to take and they are going to change his food from the Science Diet CD to another prescription food, not sure which yet.

is he getting the UTIs because of crystals in his urine? our cat has that, though he's not gotten quite as sick as yours either time he's been near-blocked (knock wood).
what's helped us is 1) low magnesium wet food and 2) this powder we put on his food to change the pH of his urine so the crystals don't form. both times he's been sick, we've also ended up (after trips to the vet and prescriptions for muscle relaxants and antibiotics) squirting water in his mouth with needleless syringes.
if this is a crystal thing, talk to your vet about the powder -- they make both pH+ and pH- versions, depending on what kind of crystals are involved. i'm convinced it makes a big difference. we put him on it after the first attack, and the second attack only happened when we had (long story) lowered his dose. and he's happy to eat it, since it's mostly beef powder and catnip. -
Yay!!! Great news! Let's hope the levels continue their downward trend and settle in the normal range.
Sweat Tea: That powder is interesting. It certainly cannot hurt and has the potential to really help a cat that is prone to urinary crystals. I don't have any male cats right now, but I am going to save that link. -

they say no crystals in the urine, and I know for a fact there were no crystals last time. i believe last time they said his urine was 'sandy", but no crystals. this time, definitely no crystals. i wonder if that powder would help him anyway?

OK, new problem! They still haven't taken the catheter out because his bladder is not contracting on it's own. They say if they give his bladder even a little squeeze, he pees. But, it is not doing it itself. So, they are leaving the catheter in and are giving him some meds (I can't remember the name, I was out on the street with screaming children when the vet called yesterday), that will help his bladder contract on it's own. Once that starts happening, then they will pull the catheter and then once again wait 24 hours to make sure he doesn't re-block and then he can come home with a bunch of meds to take.
They are now saying he most likely will not be home until Friday or Saturday. The blood in his urine is 99% gone, when we went last night to see him, it looked like pure urine and he once again looked even better than the day before.
I also spoke to the doctor about doing periodic urinalysis on him to try to combat this happening again in the future (thanks for that idea!). Figure if he gets tested every couple of months, they could see if something is coming and we can be pro-active and maybe nip it in the bud before it becomes a big traumatic event like this again.
The vet thought that was a great idea, so, from now on, I will be taking a urine sample to the vet every 2 months or so to be tested. I also think he needs to eat wet food, which he really doesn't like! I am going to have some hard time getting him to eat wet, but, i am going to try.
I also think my cat is just predisposed to this stuff! I have had male cats my entire life (the cat we had before him lived to be 14), and this is the first cat I have ever had who had this problem! -
Good luck with your cat, longtimesloper. It sounds like he is on the road to recovery, I hope the meds work on his bladder.
I know it's really heartbreaking and stressful when your pet is sick. Sometimes you feel so helpless since you are at the mercy of the vets and the medication working with your pet's system.
My cat does not eat wet food either. But I have gotten her to eat roasted chicken. So, if you have trouble getting him to eat wet food, try that. It may work better if you actually eat the chicken too. -
He ADORES chicken! the one pleasure there has been this last week with him being gone is being able to eat a piece of chicken in peace-LOL!

BTW, my cat eats ham as well. She only eats it when I hand feed it to her, not when I put it in her bowl. -

thanks for the tips, but, he wouldn't be allowed to eat those foods. He is going to have to continue being on prescription food that supposedly helps with UTIs, they are just going to switch him to something other than the Science Diet CD he has been eating for 4 years (which obviously didn't help him one fucking bit!!!) -
Great news that he is almost ready to come home. Did his kidney enzymes come down further?
I agree with you that certain cats are predisposed to blockages. I also have had many male cats through many years and only one became blocked.
I am a firm believer in (except in the case of illness) a finicky cat will eat when it is hungry. A cat will not allow itself to starve when there is food readily available. If your cat is prescribed wet food and he does not like it, mix bits of something he does like into it (like chicken, the water or oil from a can of tuna, few slivers of ham, a few pinches of liverwurst) just to get him started. Give the cat 30 minutes to eat it, then pick it up. It will dive into the next meal. They just have us sooooo well trained to offer a buffet until they eat something.

Well, I once tried wet food, and honestly, the cat did not eat for 2 days. Dead serious, it just sat there for 2 freaking days! i finally got fed up and put his dry food out, and, yep, he gulped it down. I remember I kept saying to hubby, well, he is gonna eat it eventually, if I don't give him anything else. nope, not my darn cat, 2 freaking days!!!
The vet did call me a little while ago to tell me that yeah! the meds were working and the bladder was now contracting by itself! the vet said this was the first time since my cat has been there that he had an actual empty bladder! Woohoo! Vet said that was very good news! They are going to pull the catheter tomorrow morning and if all goes well, he will be home on Saturday evening!
As for the kidney enzymes coming down more, i totally forgot to ask! they had said the other day that if he was still there Thursday, they would do another kidney level work up on me. the vet didn't mention anything to me about it and until just a minute ago, I forgot about it myself. I will ask when I go to visit him tonight.
